You sit down to think and there’s nothing there. Or worse — there’s everything there, all at once, a dozen half-started thoughts talking over each other so you can’t hear any of them finish. You walk into a room three times and forget why each time. Someone asks a simple question and your mind goes to static. People call this brain fog, and if you have ADHD, you probably call it a personal failing. Here’s the reframe that changes it: for a lot of ADHD brains, the problem isn’t that the mind is empty. It’s that it’s overcrowded.
Quick answer: “Brain fog” isn’t a diagnosis — it’s a description, and a nonspecific one. That same can’t-think-straight feeling is a recognised symptom of many treatable conditions (thyroid problems, low iron or B12, poor sleep, perimenopause, depression, anxiety, medication effects, post-viral illness), so anything new, sudden, or worsening is worth a doctor’s visit first. But when it’s part of a lifelong ADHD pattern, the experience is often less like fog and more like noise: too many unfinished thoughts at once, a mind that won’t settle. For others it’s the opposite — a slow, dreamy, spaced-out feeling researchers now study as Cognitive Disengagement Syndrome. Neither is a moral failing, and the fix isn’t “concentrate harder” — it’s getting thoughts out of your head and lowering the load.

📗 What is ADHD brain fog?
ADHD brain fog is the everyday, non-clinical name for the cloudy, unfocused, can’t-think-straight state that many people with ADHD experience. It isn’t a formal diagnosis or a single symptom. For many, it’s better described as mental noise — too many thoughts happening at once, none of them finishing — reflecting the internal restlessness and excessive mind-wandering documented in adult ADHD. For others it looks like the slowed, dreamy, “spacey” pattern researchers study as Cognitive Disengagement Syndrome. Because the same foggy feeling is also caused by many treatable medical conditions, new or worsening brain fog should be medically evaluated rather than assumed to be ADHD.
First, rule out the causes that aren’t ADHD

I’m putting this before the reframe on purpose, because it’s the part that actually protects you.
Brain fog is a real experience, but it is a nonspecific one — a study in a neurology journal found that “brain fog” isn’t one condition at all, but a colloquial umbrella covering a whole range of overlapping states, a patient-coined description that turns up across very different conditions, from long COVID to dysautonomia.¹ ¹³ And that same foggy, unfocused feeling is a recognised symptom of a long list of conditions that are treatable once they’re found:
- Thyroid problems — hypothyroid brain fog (fatigue, low mood, memory and thinking difficulty) often shows up before the thyroid diagnosis, and often eases once it’s treated — though for some people it lingers even after thyroid levels are corrected, which is itself a reason to get checked rather than guess.²
- The menopause transition — this hormonal shift is linked to measurable dips in verbal memory and thinking, which is part of why so many women describe brain fog in midlife.³ (We cover this in ADHD and menopause.)
- Poor or fragmented sleep — even short-term sleep loss significantly impairs attention and working memory, reproducing the exact brain-fog profile.⁴
- Iron or B12 deficiency and anaemia, depression and anxiety, medication side effects, and post-viral illness including long COVID — all well-recognised causes of the same cloudy state.
Here’s the rule worth keeping: if your brain fog is new, came on suddenly, or is clearly getting worse, see a clinician and ask for a basic work-up — thyroid function, ferritin/iron, and B12 are common starting points — before assuming it’s just ADHD. Lifelong, stable “I’ve always been a bit foggy and scattered” is a different story from “this started three months ago.” This article is educational, not a substitute for that check.
It’s not fog. It’s noise.

Now the reframe. When people picture “fog,” they imagine emptiness — a blank, slow, nothing-there haze. And for some ADHD brains that fits. But for a great many, the truer description is the opposite of empty. It’s crowded. The mind isn’t a blank screen; it’s a room where everyone is talking at once and no sentence gets to finish.
You can feel the difference. It’s not that you have no thoughts — it’s that you have too many, arriving faster than you can process them, each one interrupting the last. By the time you reach for one, three more have shoved in front of it. What reads on the outside as “spacing out” is, on the inside, a traffic jam. The lights aren’t off. The intersection is gridlocked.
This distinction matters because it points at a completely different fix. You can’t clear an empty fog by concentrating harder — and you certainly can’t quiet a crowded one that way. Concentrating harder just turns up the volume. The move that works is the opposite: get the thoughts out of your head, where you can lay them in a line instead of hearing them shout.
Where the noise comes from

There’s a real, if still-emerging, science underneath the noise, and it starts with a fact most late-diagnosed women find quietly validating: in adults, hyperactivity often moves inward. The child who couldn’t sit still becomes the adult whose mind can’t sit still. Researchers measuring this “internal restlessness” find adults with ADHD score significantly higher on it than their peers — the restlessness didn’t disappear with age, it relocated from the body to the head.⁵
A big part of that inner restlessness is mind-wandering — and not the pleasant, chosen kind. Studies find that the mind-wandering linked to ADHD is specifically the spontaneous, uncontrolled kind: thoughts that arrive and pull you away whether you invited them or not.⁶ There’s even a validated questionnaire built around exactly this experience, the Mind Excessively Wandering Scale, whose items describe it perfectly: thoughts constantly on the go, flitting from topic to topic, several arriving at the same time. Adults with ADHD score higher on it, and that mental over-activity independently predicts how much daily life is affected.⁷
Some researchers describe the sharper end of this as racing thoughts — and emerging work in adults with ADHD suggests racing thoughts are distinguishable from ordinary daydreaming and are tied to emotional intensity.⁸ It’s early evidence, one study, so I won’t oversell it. But if your fog feels less like a slow haze and more like a mind sprinting in twelve directions, there’s a real phenomenon there, and it has a name.
Underneath, one plausible mechanism is simply capacity. Working memory — your mind’s small desk for holding things “in progress” — has clear, documented limits in adult ADHD.⁹ When more thoughts arrive than the desk can hold, they don’t wait politely; they pile up and slide off. The felt sense of that overflow is exactly what people call fog.
The other kind: when it really is slow, not loud

Honesty requires the other half of the picture, because brain fog genuinely isn’t one thing. For some people — and sometimes for the same person on different days — it isn’t a mind racing too fast. It’s a mind running too slow: dreamy, spaced-out, staring into the middle distance, thoughts moving like they’re wading through syrup.
Researchers study this slowed, foggy pattern under a name that changed recently: Cognitive Disengagement Syndrome, formerly called Sluggish Cognitive Tempo. In 2023 an expert work group formally proposed the new term to describe the cluster of daydreaming, mental fogginess and confusion, slowed thinking, and a “spacey” quality.¹⁰ A few things to be precise about, because this is where misinformation thrives: Cognitive Disengagement Syndrome is an emerging research construct, not an official diagnosis — it isn’t in the DSM — and while it overlaps with ADHD’s inattentive symptoms, research consistently finds it’s separable from ADHD, its own distinct pattern rather than a sub-type of it, and it was first characterised in adults only fairly recently.¹¹ ¹²
Why does this matter for you? Because if your brain fog is the slow, dreamy kind rather than the loud, racing kind, you’re not imagining it and you’re not “the exception” — you’re describing a pattern researchers are actively working to understand. And it’s one more reason the “just concentrate” advice fails: you can’t will a disengaged mind back online any more than you can force a crowded one to quiet.
Why “just concentrate” was never the answer

If brain fog were a willpower problem, effort would fix it — and you’ve run that experiment more times than you can count. It doesn’t work, and now you can see why. If the problem is noise, concentrating harder just amplifies it. If the problem is disengagement, straining to focus fights the current instead of redirecting it. And if the problem is actually a medical one — thyroid, sleep, iron — then no amount of mental effort touches the cause at all.
The theme running through everything that does help is the same one from the rest of this site: stop trying to fix it from inside your head, and start moving the load outside of it. The relevant cognitive science calls this cognitive offloading — using external tools and actions to reduce the demand on your overloaded internal system.¹⁴ You’re not cheating by writing everything down. You’re using the correct tool for a capacity problem.
7 ways to quiet ADHD brain fog

Two ideas run through all of these: rule out the medical causes first, then lower the load rather than forcing the focus. Pick one or two to start.
- Do a morning brain-dump. Before the day starts, empty every looping thought onto paper — tasks, worries, half-ideas, all of it. You’re not organising, you’re evacuating. Getting thoughts out of your head and into the world is the single most direct way to turn down the internal volume.
- Park one thought, close one tab. When a new thought barges in mid-task, write it down in one line and let it go — the way you’d close a browser tab. Captured thoughts stop shouting for attention; the shouting is mostly them afraid you’ll forget them.
- Say the next step out loud. When the noise makes the path invisible, speak just the very next action — “open the laptop” — aloud. Naming one concrete step cuts a single clear line through the crowd.
- Walk without headphones. Give the noise somewhere to go. A short walk with no extra input lets the over-full mind settle and sort itself, and gentle movement genuinely supports ADHD focus (more in our piece on exercise for ADHD).
- Protect your sleep like it’s medicine. Because it nearly is. Sleep loss reproduces brain fog all on its own, so on foggy stretches, treat sleep as the first lever, not the last.
- Reduce the inputs. A crowded mind plus a crowded environment is too much at once. Cut the number of open tabs — real and mental — the notifications, the background noise, and give the signal a chance to rise above the static.
- Rule out the body before you blame the brain. If the fog is new or worsening, book the check-up and ask about thyroid, iron, and B12. Clearing your head sometimes starts with a blood test, not a productivity system.

The crowd is also the orchestra

Here’s the part worth carrying out of all this. The same noisy brain that loses your keys and drops the thread mid-sentence is also the one that connects ideas nobody else would put together — that leaps sideways to the unexpected link, the original angle, the thing everyone else walked past. The crowd in your head and the creativity are not two different features. They’re the same one, seen on a good day versus a hard one.
The goal was never to silence the room. It’s to help the crowd form a line — to go from everyone shouting at once to voices you can actually hear, one at a time. Some days that’s a blood test. Some days it’s a brain-dump and a walk. It’s never “try harder to think.” You were never bad at thinking. You were doing it in a very loud room, with no one having told you that you could open a window.

Can ADHD cause racing thoughts?
Emerging research in adults with ADHD suggests racing thoughts are a real and distinguishable experience, tied to emotional intensity and separate from ordinary daydreaming. Many adults with ADHD describe a mind that sprints in several directions at once. It’s worth knowing that racing thoughts also occur in anxiety and in mood conditions, so if they’re intense, persistent, or distressing, it’s worth discussing with a clinician rather than assuming ADHD is the whole explanation.
Is brain fog the same as ADHD paralysis or shutdown?
No — they overlap but they’re different. Brain fog is about thinking feeling cloudy or crowded. ADHD paralysis is about being unable to start despite knowing what to do. ADHD shutdown is a more complete nervous-system freeze. They can feed each other — a foggy mind makes starting harder — but the fix for each is different, which is why naming which one you’re in helps.
Why is my brain fog worse some days than others?
Because its biggest drivers fluctuate. Sleep, stress, hormones, illness, and how much you’re asking your working memory to hold all change day to day, and each of them moves the fog. A loud, crowded-mind day often follows a poor night’s sleep or a high-input, high-stress stretch. This variability is normal and is also a clue: tracking what precedes your foggiest days often points straight at the lever most worth pulling.
